News18India has the joint note on UP train accident
The findings in the document seen by AnyTV confirm the problem in the track that led to the accident. The findings say the rail track was not laid correctly and was not functioning properly. The IMR fault was detected on the line and a caution order was issued for restricted speed of 30 kmph at around 1:30 pm and was received by the station master Motiganj at 2:30 pm.

According to the loco pilot’s statement, the speed of the train on the section was around 80 when he felt a sharp vibration and a “rattling sound”. The joint note states that the engineering department should have secured the track until a caution order was received and hence they are responsible for the accident. The note also has a dissent where an official from the section does not agree with the findings.
