Question: In case of breast cancer, is the breast of all the patients cut and removed?
Answer- After breast cancer, some part of every patient’s breast has to be removed in surgery. Depending on the disease, some patients may have to remove 20 percent, some 35 percent and some may have to have 100 percent of their breasts removed.
Question: Can the patient herself get the entire breast removed to prevent recurrence of cancer?
Answer- This is not common. Doctors decide this on scientific basis. They do not do this that if the patient is afraid, the entire breast is removed as a precaution. Nowadays the treatment is better and only that part of the breast is cut and separated which has cancer cells. This is determined by looking at the size of the tumor in MRI, ultrasound, PET scan etc., then taking a margin of one centimeter. Then this part is sent to pathology, where a complete examination is done. Since cancer cells can be anywhere else in the body, away from this part, in such a situation, surgery is not the only treatment for breast cancer, but radiotherapy and chemotherapy are given to destroy those cells.
Whether the entire breast is removed or a small part is removed and radiation is given, it has been observed that the survival rate is the same for both. Therefore, breast conservation is given priority so that along with curing the disease, the patient’s quality of life can be improved.

Question- What is done after breast removal?
Answer- Breast conservation is done during mastectomy or breast removal. That is, the part of the breast which is left is preserved and breast reconstruction is done in the part which is empty. This reconstruction is done during breast cancer surgery itself, so that when the patient wakes up from sleep, she gets her breast as complete as before.
Question- How is breast reconstruction done?
Answer- Breast reconstruction is done in many ways. If 25% of the breast tissue is removed, then the remaining breast tissue is improved in a round shape and reconstructed. Its size is a little smaller than the other one but it is not very noticeable. On the other hand, if 50% of the breast is removed, then the laterodorsi muscle is taken from the muscle behind the patient’s waist and the gap of the breast is filled by bringing the skin inside and there is no gap behind the patient, the skin closes there.
If more tissues are needed, we bring skin from the back and put a silicone implant inside it and then give it the shape of a breast. This is usually done for those who do not get radiation. For some people, breasts are made from their own tissues. So, the hanging fat on the stomach of such patients is lifted along with the artery and vein and is placed in place of the breast by connecting it to the artery and vein. This is also called DIEP flap.

Answer- Its advantage is that it improves the life of the patient and when she goes back home, she feels normal. She looks and wears clothes like normal people. Her confidence remains intact. There is positivity towards life.

Answer- Nowadays, all insurance companies cover the cost of breast reconstruction along with the treatment of breast cancer, because it is not a cosmetic surgery, it is a surgery after the disease. But still, it costs between Rs 2 lakh and Rs 10 lakh. The biggest expense is in reconstructing the breast by taking fat and connecting it to the artery-vein.
Question: Is there sensation in the reconstructed breast?
Answer- If the entire breast is removed then there is no sensation but if some part is removed then the part which is left still has sensation. However, a nipple can also be made in it after 6 months.
