A health scheme run by the central government is called Ayushman Bharat Yojana. Actually, when you apply for Ayushman card, you need some documents. For this, the applicant’s Aadhaar card, ration card, residence certificate and an active mobile number are required.

You will first have to go to the public service center and meet the concerned officer. These officers work for the application, after which you will be asked for your documents.
