In order to ensure the quality of medical studies, the National Medical Commission (NMC) has prepared a comprehensive pattern of medical internship, which will be implemented from this session. While the new rules lay more emphasis on training in community health centres, efforts have been made to ensure that new physicians who start their medicine after internship are capable of performing all types of general health care and handling emergencies.
NMC has issued notification of Compulsory Recurring Medical Internship (CRMI) Regulations 2021 and these are deemed to be effective from the date of notification. Hence, it will be applicable to whatever next internship cycle comes. Only after internship the students will get the degree and will be granted permanent registration. All the students studying in the country and studying abroad will have to complete it. The annual number of such students is more than one lakh.
There will be equality in the rules
On the new rules of internship, Professor Jugal Kishor said, this will bring uniformity in internship rules across the country because at present there is little difference in every state. This is a good step. But at the same time the government should also ensure that residential facilities are also available for posting of students in community health centers and primary health centres.
compulsory internship
Students will have to undergo training for three months in community health centers and primary health centers.
During this, three weeks will have to work in the Department of General Medicine and General Surgery, three weeks in Obstetrics and Gynecology and three weeks in Community Medicine.
Specialized training in subjects such as radiation diagnostics, laboratory medicine or geriatric medicine.
One week will have to acquire practical knowledge of any one of the Indian systems of medicine like Ayurveda, Yoga, Unani, Siddha, Homeopathy.
– Existing system: At present, students have to take training for two months in community medicine, but there is no compulsion to stay in the centers, although they have to go there.
general medicine
According to the new program, students will have to work in the General Medicine Department for six weeks i.e. one and a half months. During this time he will have to be posted in the outpatient department and inpatient department and understand the working there. They will also be deployed in ICU and HDU units.
– Current arrangement: There is a two-month deployment in General Medicine, which includes 15 days of posting in the Psychiatry Department.
psychiatry department
Two weeks training will be required. In which training of patients coming to OPD and handling in emergency situations are included.
– Existing system: Including general medicine as well as 15 days training.
Department of Pediatrics
– Three weeks will have to work in the pediatric department. Which includes deployment from OPD, IPD to Neonatal Care and ICU, HDU units.
– Existing system: It is necessary to work for 30 days in the Pediatric Department.
general Surgery
– Have to work for six weeks. Mainly there will be deployment in small and big operation theaters and OPD, IPD, ICU, HDU patients will have to be looked after.
– Existing arrangement: General surgery should have a two-month posting, in which 15 have to work in the Department of Anesthesia.
Anesthesia & Critical Care
Two weeks training will be required. There will be deployment in operation theatres, intensive care units, basic life support, pain relief centers.
– Current arrangement : 15 days training is included in General Surgery only.
Family Welfare, Family Planning and Obstetrics
– Internship duration of seven weeks. Training will be given in the delivery department.
– Existing system: Two months training has to be taken in family planning.
Physical Medicine, Rehabilitation and Orthopedics
Two weeks training will be required.
– Existing system: There is a provision of 30 days for orthopedics. It has been given a comprehensive form by adding physical medicine and rehabilitation.
Emergency / Casualty
Two weeks training will be required. There is no separate training period in the present system.
Forensic Medicine and Pathology
One week training. Including autopsy.
– Current arrangement: Forensic medicine is told along with many others during the elective posting of 15 days.
skin disease/sexual disease/leprosy
One week training. In the present system, this type of training is done during the elective posting.
